Imagine you're a frail old lady and you're afraid you're going to be told you can't drive any more, as a result of this. That's how upset she was. :-)

foot hit the accelerator instead of the break scenario

If that was the deciding factor then there should be heaps of people that shouldn't be driving any more young or old.

Accidents happen and if she should lose her licence hopefully it's because of the right reasons and not based on one accident.

my Pa had an incident like the accel instead of brake

he was working on the farm in the shed, jumped in his old beat up VL, drove off, heading down a dirt road coming up to a T intersection onto the main road near his house, went to brake, hit the accel, realised instantly, went to brake again, hit the accel hard, thanks to his days of former hooning, he managed to just miss an oncoming car, turns out a piece of string (to tie hay bales with) was caught around his shoe and and caught in the door, letting his foot only good as far left as the accel, not the brake. close call.
4 wheels move the body, 2 wheels moves the soul.

Dammit, I'm sitting too close to my gf's desk. Opened to the illustration of this cross-stitch pattern, filling the page:
