As such, I thought I might set up a thread with the fixes that I have found that seem to be working for people.
1. In your cod4 directory, locate the folder called 'miles', in here, there should be a file called 'mssmp3.asi', change this file to 'mssmp3.bak' and try launch the game, this has worked for some people.
2. Issue can also occur with people running realtek audio drivers. To attempt to workaround this potential clash, go to your control panel then select: hardware and sound > sound > Recording and in here see if you have an option to enable stereo mix.
3. If your using a USB headset, simply remove the USB connection from your computer, reboot the computer, then plug the USB jack back in, as this may resolve any usb driver conflict that may be occuring.
4. If you have had a mic plugged into your computer at anystage previously plug the mic into your computer (Whether it be USB or 3.5mm connection is irrelevent) and try launch cod4 and see if a crash occurs.
5. Attempt to reinstall the audio drivers, easiest way these days is to insert the disc from your mother board, often this disk will have an option to install realtek drivers (if your running a realtek device).
These are things that have worked in the past for myself and others that I have seen with the issue.

Also, if your continuing to have crashes after this, please check event viewer, as hopefully that will show some critical logs for the crashes. If your running windows 7, then simply type event viewer into the search function, or if running another version, then go to control panel and it should be under administrator tools.
Filter the list to criticals and errors, and see if there is anything showing for iw3mp.exe. As this will help us to determine what is actually causing your crashes
