Watch some replays of people who are good. Watch how they play and learn the maps.
The other thing is your ping is massively important. With a 200ms ping you are 0.2s behind the action of the server. This gets magnified with Peer to Peer networking as seen in MW2 and MW3, where you are at the mercy of the lowest ping in the game unless you are host. Hence the reference to host advantage.
The good players can notice the difference between a 50ms ping and a 90ms ping. Really sharp players can notice the lag time for the registering of a mouse click plus the ping of the game to the server (usually the response of the mouse to the game is about 10-30ms with a decent laser mouse with good switch gear) which can tip the balance against you again. Minimising that lost time can mean the difference between frag and fail.
If you are getting surprised by shotgunners, probably need the sound turned up a bit or proper surround sound headphones. They are also a fundamental part of the game. TBH seeing an enemy is only part of the battle. You need to react to the sound just as much if not moreso.
Though the ultimate piece of information is the map itself. They give so much information on movement and potential danger areas that learning to use the map properly is just a required capability to survive ingame. This is made far harder by the **** networking of MW2 and MW3, which can be seen in direct comparison to the map workings of Black Ops, World at War and MW1.
